When using care for the first time a child / children cannot be
accepted until the approved enrolment form has been completed and
signed by the parent or guardian. Preferably complete and return
the form prior to the commencement of care. This gives the staff
time to become familiar with the child's details. This information
should be updated regularly so all details are current. Staff should
be notified of any changes in contact details, health (e.g.: onset
of asthma) or needs immediately. Should a child suffer from asthma,
an Asthma Management Plan must be completed before commencing or
continuing care.
Proof of current immunization status must be
produced for photocopying at enrolment. If the child is not immunized,
a written statement to this effect must be provided. Exclusion will
apply at times of contagious disease outbreaks.
